Temporary care of a loved one who needs continuous monitoring to give a regular caregiver a break

Home safety & medical equipment

Health Watch

Vital sign screenings and blood glucose readings, every Tuesday from 10:30am – 11:30am at the Elders Complex #1

Fall Risk Assessments

Education on fall prevention, home safety assessments, strength and balance assessments

Medication Management

Education & assistance with medication, including use of pill caddy and medication administration

Medical Transport

Transportation to medical appointments upon availability with minimum 24 hour notice


Tobacco Treatment Services

A one-on-one interview assessment is given over the phone to assess the level of tobacco use and help determine the most appropriate treatment course. The CTTS then works with the client to develop a customized quit plan.

Nutritional Outreach:

The Michigan Tribal Food Access Collaborative and the Walmart grants are in place to help improve access to healthy foods and enhance nutritional landscape in tribal communities. The health educator can help with nutritional resources and education to patients in need.

Comprehensive Cancer Navigation:

The Three Fires Comprehensive Cancer Consortium grant is in place to provide monetary help to those navigating through their cancer journey. It also provides an aim to decrease colon and lung cancer, improve quality of life among cancer survivors and decrease cancer morbidity in American Indians.

Breast and Cervical Cancer Navigation Program:

The BCCCNP is a program to help uninsured or underinsured women have access to life-saving cancer screening services and follow-up care, including cancer treatment if that should be needed.

Hannahville Farmers Market:

The Hannahville Farmers Market operates every Thursday from 11:30-3:30 EST beginning in June and runs through September. The Hannahville Farmers Market accepts SNAP, DUFB, WIC and Senior Project Fresh benefits.

Home Visiting Network in the U.P.

Hannahville MIHP (Maternal Infant Health Program)

Home visiting program for Native American pregnant mothers or Native American infants 18 months and younger with FFS Medicaid. MHIP offers support and education to provide healthy pregnancies, positive birth outcomes and healthy infants. Services are provided by a Registered Nurse, Medical Social Worker, and Registered Dietitian.

Prenatal through 18 months.

Native American tribal member or descendant with FFS Medicaid. Does not need to be Hannahville Tribal Member, just live in service area.

Hannahville Healthy Start/Family Spirit Program

Home visiting program for Native American families including mothers, fathers, infants and caretakers of Native American children.  Staff provide education and support services to provide healthy pregnancies, positive birth outcomes, and healthy infants with our main goal being to reduce infant mortality among Native Americans in Michigan. We follow the Family Spirit Native American Curriculum.

Prenatal through 2 years of age

Pregnant mother must be carrying Native baby or be Native American herself, child must be Native American and live in service area.  Does NOT need to be Hannahville Tribal Member.
